Read the following seven items carefully before agreeing.

ATTENDANCE EXPECTATIONS: Company dancers are committed to attending regularly and being punctual. Ideally, your child will only miss 1-2 classes for the entire year. Missing two classes in a row, or any absence in the months of January - April, will require a private lesson before your child’s next class. Private lessons are $5 each and are given by a teen dancer.

PRACTICE EXPECTATIONS: Company dancers are to be prepared for class each week, having practiced at home using videos and music on their portal. While we expect very little from our Preschoolers, our expectation for preparedness increases with age. Note that outside practice is encouraged but not required in our Recreational and Acro classes. *

DROP-OFF PROTOCOL: Please do not use us as a babysitting service. If you are dropping off your dancer, they may be dropped off 10 minutes before class begins and picked up within 10 minutes of classes ending. If your dancer, under 13 years of age, is taking multiple classes and has a wait in between, contact us for approval that they may wait unsupervised. We are trusting that our teenage dancers will wait in the appropriate area with appropriate behavior. All dancers must stay in the waiting area (the yellow chairs or white couches). If dancers are found wandering / playing outside the waiting area, you will be required to escort them in and out of the classroom in order for them to continue taking classes with us. *

STUDIO PROTOCOL: No entry is permitted prior to 4:00pm. If your family is waiting onsite, all family members must stay in the waiting area (on the yellow chairs or white couches) and children be supervised at ALL times. Playing or wandering in any areas other than the waiting area is not permitted. To remain in good standing with Crossroads Church and to be good stewards of what has been entrusted to us, note that families leaving any children unsupervised inside or outside of the building are subject to immediate dismissal. No fees or past months' tuition will be refunded.

SPRING SHOW PARTICIPATION: In order to participate in the Spring Show, dancers must attend all mandatory rehearsals in their entirety. Until the show date and rehearsals have been secured, please keep your spring schedule open. Please do not schedule any vacations within six weeks of the show. All tuition and fees must be paid in full. *

DRESS CODE REQUIREMENTS: Dancers are to attend classes in the full uniform that is designated for the current season. Rentals are available if a piece is forgotten. Hair is to be secured off of the face and neck. For the Spring Show, dancers must abide by the dress code set for their group. Until this is announced, please avoid short haircuts and dying hair an unnatural color. Because all jewelry is to be removed, please avoid first-time ear piercing around showtime. *

COMPLETING THE DANCE SEASON: Company dancers are committed to finishing the dance season through to April and tuition is due for the entire year, regardless of attendance. If, in the past, your family has dropped out before April and you would like to register the following dance season, you must pay the entire year’s tuition up front (non-refundable) to secure your spot. *

If I cannot be contacted, I authorize Grace Christian Dance Company, LLC to seek medical services in case of serious injury or illness. I further agree to accept financial responsibility in excess of the benefits allowed by my health plan.

I am aware that participating in any dance classes and acrobatic dance/tumbling (acro-dance) involves inherent risks and hazards. I freely accept and fully assume all such risks, dangers, and hazards and the possibility of bodily or personal injury, property damage, or loss resulting from such risks and hazards. I voluntarily agree to release Grace Christian Dance Company, LLC, Grace Christian Dance Company, LLC’s employees/volunteers/contractors and Danielle Black from any and all liability for any loss, damage, injury or expense that I or my next of kin, successors or dependents may suffer or incur as a result of participation in classes, camps, demonstrations, performances or athletic activity due to any cause whatsoever.

I also hereby grant Grace Christian Dance Company, LLC permission to use my/my child’s likeness in photographs and videos. These images may appear on gracechristiandance.com, Facebook, Instagram or any promotional materials used in association with Grace Christian Dance Company, LLC. The students’ names and personal information will not be printed. I understand and agree that these images will become the property of Grace Christian Dance Company, LLC and I hereby irrevocable give permission for Grace Christian Dance Company, LLC to edit, copy, exhibit, publish or distribute these images for any lawful and moral purpose.
